CONSENT TO PHOTOGRAPH, FILM AND USE IMAGERY

I hereby grant permission to MVP Basketball Pty Ltd to use photographs and/or video of me taken when I or my child attend the MVP Basketball Camp or training sessions.

I acknowledge and consent to any film or imagery of my child that was taken when I attended a training session or camp provided by MVP Basketball may be used in publications, news releases, online, and in other communications related to promotion of MVP Basketball’s business.
